DMV Placard

We are happy to provide a DMV placard for all patients who qualify for it.  

A few tips:
1) Make sure the form is filled out perfectly.  DMV is notorious for denying them because of small errors.
2) If you have AAA auto club, go there to submit the form.  It's much faster.

We have forms ready for you with my information pre-printed on it.  The link is password protected so you will have to contact my office for the password.  You will have to mail us the form with a SASE (self addressed envelope) because the form requires a doctors ink signature.  No stamped signatures are accepted.  You can all bring the form to the office and we can sign it during your visit.
